Our family support and case management empowers families, individuals, and the communities in which they live. This is achieved through:
- Listening to families
- Providing information and referrals for a variety of needs
- Giving family assessments, with awareness of:
- Family safety and health, including access to health care
- Risks of family/domestic violence
- Providing family casework/case management
- Programs focused on enhancing economic stability:
- Educational programming
- Pregnancy Services
Services are available to everyone regardless of race, ethnicity, language, place of residence, mental health or illness, physical health, ability to pay for services, country of origin, faith tradition or immigration status. We build on the strengths of family members to assist you in meeting your own needs. We follow these practice standards: 1. We empower families to overcome barriers that restrict their personal and community growth 2. We empower families to advocate for themselves and on behalf of others in the neighborhood 3. We connect families to the resources they need to succeed and raise healthy and successful children 4. We support the capacity for families to improve their economic stability and success 5. We foster partnerships and build relationships within the community via neighbors, parishes, volunteers, or other community structures 6. We encourage input from participants both initially and in an ongoing way in the development of program goals, program design, and redesign 7. We collect and measure outcomes that reflect best practices or promising and emerging practices in the field (Adapted from Annie E.
